Since 1978, Bluegrass Care Navigators (BCN), formerly known as Hospice of the Bluegrass, has provided physical, emotional and spiritual care to thousands of seriously ill patients and their families in Kentucky. Care is delivered by a multidisciplinary team which includes the patient’s physician as well as nurses, social workers and chaplains. Six offices and three inpatient units across the state assure access for anyone in need of care and support during all phases of terminal illness. Bluegrass Care Navigators is a member of the National Hospice and Palliative Care Organization, is certified by Medicare and Medicaid, is licensed by the Commonwealth of Kentucky and accredited by The Joint Commission. Bluegrass Care Navigators is a 501(c)3 nonprofit organization.
